I can't remember the details of this.  I have seen these pictures several times in my mining retraining classes for surface/underground.

 

I do not believe the driver of the truck was killed in this one.  I think the mechanic parked his truck, got into the rock truck and drove over his own truck.  If my memory serves me right...

 

There was another one with a red chevy 1500 that the driver was killed...

 

Not trying to be a downer..just sharing..
