I can't answer your question as to why called different things for same designation, but here is a pic of my '68 SS/Super 12.

Has "Super 12" on the hood, SS12 on grill.

I was always told It stood for "super suburban"

From what I have read the Super 12 has larger tires and the cig lighter other than that I dont see any other major differences.

Super (SS) 12 - Wood grain dash, cig lighter, longer footboards, 16x5.50-8 front tires, 23x8.50-12 rear tires
Suburban 12 - No wood grain, no cig lighter, shorter footboards, 4.80-8 front tires, 22.5x7.50-12 rear tires

I don't know if they are original to it but my ss12  has short foot boards. It has the aluminum dash tower, cig lighter, no clutch safety switch and hood inserts. I think that they are original since it would be hard to pull the clutch peddle out just to change the foot boards. Mine also never had a tool box and the pto slide that came with it had a model plate on it, neither of my other ones have one.
